Sage Care Management begins each new relationship with a thorough needs assessment of  your specific situation. Every circumstance is different, so we take great care in gathering information and leave no stone unturned to get a total picture of your loved one’s needs… from home, medical and dietary needs to their social and spiritual life. We identify strengths and risks, offer recommendations and create a comprehensive plan that can be carried out by the family or by our diverse team, including Certified Care Managers, Licensed Clinical Social Workers and Registered Nurses who are on call 24/7 in case of emergencies.

Sage Care Management can:

  • Conduct personalized care-planning assessments to identify problems, eligibility for assistance, and need for services
  • Screen, arrange and monitor in-home help or other services
  • Review financial, legal, or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets
  • Provide crisis intervention
  • Act as a liaison to families at a distance, making sure things are going well and alerting families to problems
